Submit a Privacy Request

Use this form to ask myCrewIQ, LLC for a copy of the personal information we hold about you, to correct it, to delete it, or to opt out of its sale or sharing. Every request is logged, verified, and answered by a human.

Request Received

We have logged your request. We will confirm receipt by email to the address you provided within 10 days. You will get a substantive response within 45 days. If we need more time, we will tell you why before that deadline passes.

Before you start. We only use what you enter here to verify who you are and to process this request. We do not add you to any mailing list, and we delete verification details once the request is closed.

03 — Details

Anything that helps us find your records Do not enter a Social Security number, a driver's license number, a bank or card number, or any health information. We never need those to process a request, and this form is not the place for them.

We verify identity before releasing or deleting anything — usually by replying to the address above, sometimes by asking you to confirm a detail only you would know. Submitting this form does not create an account and does not waive any right.

What Happens Next

Logged on arrival Your request is timestamped the moment it lands.
Confirmed in 10 days We acknowledge receipt and tell you what we need to verify you.
Identity verified We match you to our records before we release or delete anything.
Answered in 45 days You get a written response. If we need another 45 days, we tell you why first.
Closed and retained We keep a record of the request itself, as the law requires, and nothing more.
